The City provides eligible employees with a group life insurance policy at no cost. The benefit level is based on your bargaining unit as follows:

Eligible employees must complete a Group Life Insurance enrollment form to enroll and designate a beneficiary.
Employees may change beneficiaries at any time by submitting a Beneficiary Change Form to Human Resources (Form available in the "Benefit Forms" Section of the Benefit Information Page.
