Important Enrollment Information

New Employee:
As a new employee you have certain restrictions regarding enrollment into benefits. You have sixty (60) days from date of hire to enroll in medical insurance, you have thirty (30) days from date of hire to enroll in dental and vision insurance. If you miss these deadlines you may request to enroll in medical at Open Enrollment or through Late Enrollment with a 90-day waiting period from date of your request. If you miss the deadline for dental and vision you may request to enroll at Open Enrollment only.

Current Employee:
Family status changes-For medical insurance, employees have sixty (60) days from date of marriage to enroll a new spouse. If deadline is not met, the spouse and or spouse’s children may be enrolled with a ninety-day (90) waiting period or at Open Enrollment. For dental and vision employees have thirty (30) days from date of marriage to enroll a new spouse. If this deadline is not met, the spouse and/or spouse’s children may only be added at Open Enrollment.

IMPORTANT: It is the responsibility of the employee to notify Human Resources immediately upon a change in family status. These changes include: Divorce, marriage etc. If you are covering an ineligible dependent on medical, dental or vision insurance you will be financially responsible for any medical, dental or vision services the person received while in an ineligible status! Example of ineligible dependents: ex-spouses.
